TopicAlpha Rhythms as Functional Neuromarkers

Alpha rhythms (8–13 Hz) are the dominant features of resting-state EEG, particularly in the eyes-closed condition, and are fundamental to cortical self-regulation. This presentation provides a concise, practice-oriented overview of alpha physiology, distinguishing occipital alpha from Rolandic mu rhythms and explaining how thalamocortical circuits generate and modulate healthy rhythmic activity. We will translate core mechanisms—such as alpha reactivity (desynchronization/synchronization) and sensory gating—into clinically useful markers. Emphasis is placed on recognizing abnormal alpha in both frequency (e.g., slowed or unstable peak) and localization (e.g., diminished posterior dominance, atypical frontal alpha), with examples of how these patterns inform differential diagnosis and guide intervention strategies. The session concludes with a detailed patient case demonstrating abnormal alpha rhythm, step-by-step interpretation, and take-home principles for integrating alpha neuromarkers into routine assessment and treatment monitoring.
